Do you CHECK FOR CORONAVIRUS SYMPTOMS BEFORE I COME IN?

Yes, we ask a number of questions about your current health.

How do I make an appointment?

While we do not offer online appointment-making, you can call MMC or SWAHS anytime between 8:00 AM to 5:00 PM to schedule an appointment with our providers. If possible, you can come in on the same day!
 

do you accept walk-ins?

Yes. When you arrive, you will be checked in and assessed to determine your needs.
 

Who do I call after CLOSING?

You can call the phone number for MMC or SWAHS for an on-call provider.

WHAT INSURANCE DO YOU ACCEPT?

We accept most insurance and federal assistance. However, we do not accept any pharmacy cards.
 

I can’t afford my PRESCRIPTION medication. What do I do?

We understand the difficulty of trying to purchase prescriptions. A prescription assistance program has been set up with major pharmaceutical companies and BLBAHDB, Inc. to address this need. You’ll need to fill out an application and must have the following:

  1. Proof of income for the entire household. This includes one or more of the following:

    • Copy of current 1040 Federal tax return

    • Copy of Medicare benefits

    • Copy of SSI benefits

    • Copy of pension benefits

    • Copy of Food Stamp benefits

    • Copy of Disability benefits

    • Green Card/Visa/Citizenship papers

  2. Patients must see their physician before applying.
     

What about dental services?

We refer all dental patients to Southwest Mobile Health Center in Mobile, AL. Dental services are available on Tuesdays from 7:30 A.M. to 6:00 P.M. Walk-ins are welcome. Call (251) 666-7413 to make an appointment. Be sure to bring a valid form of picture ID, insurance (if applicable), proof of income, and payment for a dental fee.
